1. Location. This should be filled in with the name of the place the pay phone is at. If the pay phone is outside on a street and has no place associated with it, then check the little box below the location line and leave the location field blank.
2. Address. This should be filled in with the proper street address of the place where the pay phone is at. If you are sending in pay phones from a large instituion with no known address like a college, an airport, or a mall, use the address field to tell where the pay phone is within the large place, using different descriptions for each group of phones. If you absolutely can not figure out the address, check the address unknown box, and the the address field blank.
3A. City and State/Province (domestic form only). Enter the city and state (province in Canada) where the pay phone is.
3B. City and Country (international form only). Enter the city and country where the pay phone is.
4. Pay Phone Quantity. Enter the number of pay phones at the location you are sending. This will produce the appropraite number of blanks in the second part of this process. The maximum is 20, for places with more phones than that, send in groups of phones one group at a time.
5A. Area Code and Prefix (domestic form only). This optional field was created to save typing. If you are sending in numbers from a place with lots of pay phones, the phone number forms can be created with the area code and prefix already filled in. You can still edit the area codes and prefixes in the second part in case some pay phones have different area codes or prefixes.
5B. Country Code (international form only). This field requires you to enter the country code of the pay phone number you are contributing.
6. & 7.Your Name and E-Mail. Please enter your name and e-mail address as you would like it to appear in the small file which gives you credit for sending in the number. E-mail addresses are no longer publicly visible on this web site, and are used to identify repeat contributors. The e-mail address you enter will be used by me in case I need to contact you about the numbers you send in.
8. Remember your name and e-mail address for future submissions. This checkbox causes the program to store the name and e-mail address you give on our server. It also sends a cookie to your browser so that the name and e-mail fields will be automatically filled in if you ever visit the phone submission form again. This is strictly optional.
9. Phone numbers, incoming calls, inside/outside. These fields do not appear until part 2 of the submission process. Enter all the pay phone numbers here, and also indicate whether the phone allows incoming calls, and if it is inside or outside. If a pay phone charges for incoming calls, please answer "yes", then change the next box from "are free" to "cost $$$".
10. Phone Equipment. This field lets you tell me what kind of pay phone is being used. Not all pay phones are the same. To learn what they all mean, click here.
11. Phone Company. Fill in the name of the phone company which owns the pay phone. Nowadays, lots of different companies own pay phones, not just the local phone monopoly.
